Proactive Medicine for Your Tomorrow

For the past several years Dr. Castellanos has chosen to focus on office practice instead of hospital medicine. The goal of this practice is to help patients avoid needing the services of Emergency Room Medicine and Hospital Medicine.

As a Multi-Specialty Practice we provide our patients with the necessary information to proactively prevent and reverse disease and accelerated aging.

We accomplish this by focusing on several medical specialties for the evaluation and treatment of our patients. Our areas of focus are as follows:

Family Medicine

We take a comprehensive common-sense approach to patient evaluations.

Laser Medicine

We use Laser Medicine for the treatment of lesions, pre-cancer, and cancer in Dermatology and Gynecology. In addition, we use Laser Treatment for the removal of hemorrhoidal tissue.

Dermatology

We provide evaluation and treatment of the largest organ – the skin.

Women’s Health

We cover many aspects of a woman’s health needs including complete gynecological exams, breast exams, breast biopsies, hormone modulation therapy, cervical cancer screening with colposcopy and treatment

Welcome to Medicare Preventive Visit

•How often is it covered? – Medicare covers all the costs for a one-time, comprehensive “Welcome to Medicare” preventive visit during the first 12 months that you have Part B.
•Who’s eligible? – You can call and sign up for the visit during the first 12 months you have Medicare.
•Your costs in Original Part B Medicare – It’s free. There’s no copayment or deductible for the visit.

Yearly Wellness Visit

•How often is it covered? – If you’ve had Part B for longer than 12 months, you can get a yearly “wellness” visit to develop or update a personalized plan to prevent disease based on your current health and risk factors. This visit is covered once every 12 months.
•Who’s eligible? – All people with Medicare. If you got the “Welcome to Medicare” preventive visit, you have to wait 12 months before you can get your first yearly “wellness” visit. You don’t need to have a “Welcome to Medicare” visit before your yearly “wellness” visit.
•Your costs in Original Part B Medicare – You pay nothing for the yearly “wellness” visit. If you get additional tests or services during the same visit that aren’t covered under these preventive benefits, you may have to pay coinsurance, and the Part B deductible may apply.

Alcohol Misuse Counseling

• How often is it covered? – Medicare covers one alcohol misuse screening per year. People who screen positive can get up to 4 brief face-to-face counseling sessions per year.
• Who’s eligible? – Adults with Medicare including pregnant women who misuse alcohol, but aren’t alcohol dependant and are competent and alert during counseling.
• Your costs in Original Part B Medicare – You pay nothing. If you get additional tests or services during the same visit that aren’t covered under these preventive benefits, you may have to pay coinsurance, and the Part B deductible may apply.

Cardiovascular Disease (Behavioral Therapy)

• How often is it covered? – Medicare covers 1 visit per year with your primary care doctor
• Who’s eligible? – All people with Medicare.
• Your costs in Original Part B Medicare – You pay nothing. If you get additional tests or services during the same visit that aren’t covered under these preventive benefits, you may have to pay coinsurance, and the Part B deductible may apply.

Obesity Counseling

• How often is it covered?
o One face-to-face visit every week for the first month;
o One face-to-face visit every other week for months 2 – 6
o One face-to-face visit every other week for months 7 – 12, if the patient meets the 6.6 lbs weight loss requirement during the first six months
o Up to 22 times in a 12-month period.
• Who’s eligible? – People with Medicare who have a body mass index (BMI) of 30 or more.

Smoking Cessation Counseling

• How often is it covered? – Medicare will cover up to 8 face-to-face visits during a 12-month period.
• Who’s eligible? – People with Medicare who use tobacco.
• Your costs in Original Part B Medicare – You pay nothing for the counseling sessions. If you get additional tests or services during the same visit that aren’t covered under these preventive benefits, you may have to pay coinsurance, and the Part B deductible may apply.
